Forms

Build a form once and use it two ways: as a card inside a chat, or as questions the assistant asks one at a time in conversation. Every question maps to a data point, so answers land on the right contact automatically. Purely browsing visitors are never asked; a form appears when someone is engaging.

What you can do here

Start from a template

Pick a ready-made form (new patient inquiry, appointment or callback request, insurance pre-check, or patient feedback) and it is copied into a draft you can customize. The original template is never changed, so your edits stay entirely yours.

Predefined and custom data points

Assemble a form from a catalog of common fields (name, email, reason for visit, urgency, consent, and more) or add your own custom field. Because each question is tied to a data point, what you collect stays consistent and reportable.

Typed questions and validation

Each question has a type and a control: short text, paragraph, number, date, dropdown, radio, checkbox, multi-select, a rating, or a consent toggle. Answers are checked as they come in, so a bad email or empty required question is caught before it is stored.

Show-if logic

Reveal a question only when an earlier answer calls for it, so people see a short, relevant form instead of a long one.

Two ways to ask

The same form renders as an in-chat card with real controls, or as a friendly back-and-forth where the assistant asks one question at a time. You author it once.

Answers map to the contact

As a form is completed, answers are written onto the person's contact record: their details, interests, and consent. Sensitive clinical questions are routed to your records system rather than stored here.

Versioning and responses

Publishing a form sets its version, and every response records the version it answered, so later edits never scramble older answers. The Responses tab shows completion rate and where people drop off.

Follow these steps

Build and publish

  1. Use New form for a blank form, or Start from a template to begin from a ready-made one.
  2. Add questions from the field picker, set each one's control, mark the required ones, and drag to reorder.
  3. Watch the live preview on the right, then Save and publish when it is ready.

Learn from responses

  1. Open a published form and switch to Responses to see completion rate and per-question drop-off.
  2. Trim or reorder questions where people abandon, then publish again.

Helpful habits

  • Ask only what helps you route or qualify the person; a shorter form is completed more often.
  • Keep clinical detail light: deep medical history belongs in your records system, not the CRM.
  • Always ask for consent before collecting a channel you intend to message on.

Tips

  • Only admins publish forms and create custom fields; anyone on the team can build a draft.
  • Templates are copied into your own draft when you use them, so customizing one never changes the original.
  • An email or phone captured in a form is held as pending until it is verified, so it never merges the wrong records.
  • Required questions are enforced only when the person finishes, so a conversational form can be answered gradually.
  • Leaving the builder with unsaved edits asks before discarding them, so a stray click never loses your work.
